PLEASE NOTE: The application is a two-step process:
Part I. Fill in and submit the online application information requested in Part I by April 15.
Part II. Assemble and submit the supporting materials and signature page requested in Part II. These items should be mailed (postmarked) to the Episcopal Foundation of Dallas, 5950 Cedar Springs Rd. Suite 125, Dallas, TX 75235 or faxed to 214.352.8361 by April 15. (If April 15th falls on a weekend or holiday, you may submit materials by the next business day.)

Part II Required Supporting Materials and Signatures

Your request cannot be considered without providing the following supporting information to the Foundation:
- A narrative summary of your grant request (three pages maximum):
- Describe the work of your organization (eg. a brief history, mission statement and current programs).
- Describe the program or project for which you seek funding, including
- A statement of the program’s primary purpose or the need or problem that you are seeking to address.
- The population that you plan to serve and how this population will
benefit from the project.
- If you are not an Episcopal organization, please describe how your program or project fulfills the Christian
ministry and mission of the Episcopal Diocese of Dallas including how your program interfaces with or is supported by Episcopal organizations in the diocese.
- If this is a start-up or first-year program, what are your plans for funding and sustaining the operation?
- Discuss other possible sources of funding for the project and address whether partial funding from the Foundation
would be helpful.
- A copy of the organization’s current year operating budget with revenue and expenses.
- A copy of the organization’s prior year financial statements, balance sheet and statement of revenue and expenses. Audited statements are preferred but not required.
- A detailed project budget and supporting bids, as applicable.
Note: Requests for funding major purchases of furniture, fixtures or equipment should be accompanied by independent documentation of the cost (i.e. outside bids are preferred, online quotes, price lists). The larger the dollar amount requested, the higher the standard for documenting competitive pricing.
- A copy of your most recent IRS letter indicating your agency’s tax-exempt status or, if not available, an explanation.
- A list of your Board of Directors or Vestry with addresses and phone numbers.
- A completed signature page (download) signed by the authorized individual listed in Part I.
